Elevate is a hybrid practice. We are in-network for Medicare Part B beneficiaries and we are out-of-network for all commercial insurances.
We choose to be out-of-network for all commercial payors to provide the premium services that we offer. Click Here to read more about why insurance-based models place limitations on providing quality care.
We are happy to provide all necessary documentation for patients wishing to receive out-of-network reimbursement from their insurance company. If you are not sure whether your insurance allows out-of-network reimbursement, please send us an email with your insurance information and our team will be happy to assist in verifying your benefits.

Be sure to dress in clothing and shoes that are comfortable to move in. Set up a quiet space suitable for an evaluation. All you need Is enough space to set up a portable treatment table!
We will start by listening to your story, asking you questions, and setting meaningful goals. We will then complete a physical examination and compose a game plan to achieve your goals. By the end of your first visit, you will feel empowered, motivated, and have a better understanding of your pain and injury.

Initial Evaluation and follow up visits typically last 60 minutes. Visits may be shorter or longer depending on your condition or individual needs.

Traditional physical therapy may discharge you early for a few reasons. Most commonly, your insurance ran out of visits, the therapist was not equipped to help you get back 100%, or the therapist did not effectively communicate with you.
